Transcervical and transcervical-transparotid approaches to the parapharyngeal space (PPS) may be enhanced by mandibulotomy. However, traditional midline mandibulotomy requires lip-splitting and intraoral incisions. We report a case of pleomorphic adenoma of the PPS which was resected through the subcutaneous mandibulotomy. The tumor was 70 mm × 55 mm × 40 mm in size. The tumor was too large to be resected through the transcervical-transparotid approach. However, the additional exposure achieved by the subcutaneous mandibulotomy was adequate to remove the tumor. We recommend the subcutaneous mandibulotomy, which affords good access to the PPS without lip-splitting and offers improved cosmetic result compared with traditional midline mandibulotomy.
