Japan has achieved universal coverage and relatively successful cost containment, not through radical reform, but by making incremental adjustments. The fee schedule that defines the services covered by social health insurance and the payment to providers has played a key role in realizing policy goals. The system's sustainability will depend on whether it can meet increasing demand for quality and efficiency by devolving responsibility to the prefecture level. © 2008
