A 29-year-old nulligravida woman presented with severe dysmenorrhea and primary infertility. Hysterosalpingogram revealed a right uterine horn with a solitary patent tube. Magnetic resonance imaging revealed a right unicornuate uterus continuous with the cervix and the vagina and a rudimentary left uterine horn. Laparoscopy revealed a noncommunicating left uterine horn and a left ovarian endometrioma. The ovarian endometrioma and rudimentary horn were excised and removed laparoscopically without complications. The patient was able to spontaneously conceive a year after the procedure and delivered a healthy boy by caesarean section.
