Purpose. Dry eye with tear deficiency is categorized into two types, Sjogren's syndrome (SS) and non-Sjogren's syndrome (non-SS). To clarify whether non-SS dry eye is a precursor of SS dry eye, we have performed a prospective follow-up of dry eye patients. Methods. Twenty-two SS patients (modified Fox criteria) and 30 non-SSdry eye patients with positive serum autoantibodies (RA, ANA) were followed for 2.1±1.1 years. Tear dynamics and ocular surface, as well as the systemic autoimmune status were examined during the follow-up periods. Results. All the tear dynamics and ocular surface conditions did not change during the follow-up, as shown below. Schirmer Nasal Tes. TFI RB Fluo (mm) (mm) SS first exam 2.9±2.5 7.0± 5.7 36±126 5.8±2.5 3.5±1.9 second exam 3.0±2.9 7.9± 6.1 44±102 5.6±2.5 3.0±1.8 Non-SS first exam 3.8±3.0 21.0±11.0 61±144 2.6±2.4 2.0±1.2 second exam 4.3±3.5 20.1±10.7 94±212 2.5±2.3 1.8±1.1 Patients's auto-antibodies (RA, ANA, SS-A, SS-B) did not change in either group, except one non-SS patient whose ANA antibody increased to 160X. Conclusions. SS and non-SS dry eye did not change their types during the two year follow-ups. These results suggest that the two types of dry eye have different clinical entityes, without any conversions.
