Many methods of reconstructing the madibular bone with the use of free flap transfers have been reported. Adding to these reports, we present our clinical experience with the use of a scapular moyamoya fascial flap along with the scapular bone. Due to radiation, the patient had developed a fistel (?) fistula (?) between the skin in the madibular region and the oral cavity and the mandibular bone had become osteomyelitic. Using a scapular moyamoya (?) flap, we designed two skin paddles (?) pedicles (?) and elevated the flap with the scapular bone, with the pedicles connecting to the scapular circumflex artery and vein. In this manner we were able to reconstruct the mandibular bone with a facial flap. The patient's postoperative course has been very good. As part of our on-going investigations, we intend to pursue a histological study of facial flaps and adaptable skin regions.
